To get started, fill out an Independent Contractor Application form. Only list previous officiating experience! You can type into the form directly from your browser. When finished, print it out, sign it, and mail it to the City of Santa Cruz Parks and Rec office at 323 Church St., Santa Cruz, CA 95060. You can also fax it to 831-420-5271.
